Eligibility Requirements:Programs are for low income children with life-threatening medical conditions or developmental disabilities under the age of 18 and their immediate family members only. The child's guardian or parent should complete applications. When more than one diagnosis is involved, each attending physician must complete a medical statement. Eligibility will be determined by Heavenly Intervention Inc. after physician's assessment. Applications will be reviewed and approved by the Board of Directors. Children must live in Akron, Ohio or surrounding areas to participate in Heavenly Intervention Inc. Programs. Application |
